Insurance Billing

For covered services, SDI Labs bills the payor directly when valid insurance information is provided with the requisition.

  • Include a copy (front/back) of the insurance card
  • Ensure ordering provider information is complete and signed
  • Patient may be responsible for copays/deductibles per plan
Patient Pay

Self-Pay / Patient Responsibility

When insurance is not available or a service is non-covered, the patient may be billed directly.

  • Transparent statements; assistance available to review charges
  • Payment options provided on statement
Client Bill

Client Billing (By Arrangement)

For practices with contractual arrangements, SDI Labs invoices the client at agreed rates and terms.

Coverage

Before You Test

Verify eligibility & benefits

Coverage can vary by plan and indication. Contact our team with the patient’s payor, member ID, and test indication, and we’ll assist with verification.

Prior authorization

Some plans require prior authorization for certain molecular tests. If required, the ordering provider should initiate authorization before specimen collection.

Out-of-network

If a plan is out-of-network, services may still be available. We’ll discuss options with the provider and patient.
